What is the Follow My Health Patient Portal?
The Follow My Health patient portal is a tool that Pediatric Associates has implemented to give parents and families access to their health records. In the portal, you can see and print office notes, results done in-house (such as rapid strep tests, UAs, and fingerstick hemoglobin), and your child’s immunizations. In order to maintain patient-clinic relationships, provide safe communication through our trained staff, and continue to offer timely available office appointments, our portal does not have the functionality to message providers or office staff, request appointments, or request medication/refills. These requests can continue to be made through calling our office at 417-882-1600 or by contacting your pharmacy.

Following your annual exam, if you had sports physical or club paper work completed, these documents are readily available if you were to need copies for another school club or organization.

Access your child's immunization record for quick access for daycare or school. Official immunization records can still be requested the office.
Accessing the Patient Portal
Fill Out the Patient Portal Invite Request Form
- Portal Access must be requested for each child by each parent requesting access by filling out our Patient Portal Invite Request Form
- Request a Patient Portal Invite Request while in the office an return to the front desk staff or your healthcare team OR
- Download the Patient Portal Invite Request using the link above and bring this form to your next visit to the office, fax the form to our office, or mail to:

* Please Note: In order to be given access to the portal, the adult requesting access must be on the most recent patient's shared information form.
- Once this request form is received by our office staff, an invite will be sent from Follow My Health to the email you provided. You will then follow the instructions provided in the email to set up the portal
- It may take 3-5 business days from the time the Patient Portal Invite Request form is received by the office to receive your invite email. If you have not received an email invitation within one week of submitting the access request form, please contact the office.
- Each invite will require a security code to set up your account. This code will be the 4-digit birth year of the oldest child you are requesting access for.
- After accepting Follow My Health's terms and conditions, release of information , and consent, access should be available for each child requested.
Helpful Facts about the Follow My Health Patient Portal for Pediatric Associates
- When setting up your portal, it is helpful to choose a username different than your email which is the default entry (although the process suggests this, it can cause some confusion when otherwise logging in later)
- If you would like to receive text or email reminders about your child’s upcoming appointments, you must opt-in to these through communication settings
- Our portal does not have access to message providers, make or change appointments, or send refill or medication requests and should not be used in emergency situations
- While the portal has options to select your pharmacy and input custom data such as immunizations and vital signs, your provider will NOT be able to see these and those entries are not part of the official medical record.
- If your child is between the ages of 12 and 17, some documents may not be able to be seen in the online portal pursuant to MO Laws: MO St. 431.061(4)(a, b, c). At age 18, authorized user access is terminated for the parent but can be reestablished to the child with request.
